- If traveling within the United States, the processing fee is $12/person. There is no fee for registration only.
- If traveling internationally, the processing fee is $17/person. There is no fee for registration only.

Please complete this process no later than 2 weeks before departure so you will not incur a $2 per member late fee.
Cancellation Policy: UMVIM, SEJ will only reimburse the insurance cost for any team member cancellations not the processing fees.
HOW DO I MAKE A PAYMENT?
If paying by check, mail a check payable to UMVIM, SEJ and mail to 100 Centerview Drive, Suite 210, Birmingham, AL 35216. Please include the team leader’s name on the memo line and include a copy of your invoice with the payment.

WHAT IS MISSION SPORTS INSURANCE?
The Mission Sports coverage is the same basic coverage as the standard plus the Hazard Sports -motorcycle/motor scooter riding (whether as a passenger or a driver), hang gliding, parachuting, bungee jumping, water skiing, snow skiing, snowmobiling, snowboarding, and spelunking.
HOW DO I GET THE INSURANCE ID CARDS?
The insurance company will email the following to the group leader:
- ID cards for all team members (includes 24/7 toll-free numbers for assistance)
- Seven Corners Certificate (policy of coverage and exclusions)
- Group Leaders Field Guide (print a copy to have during the mission)
- List of Frequently Asked Questions from Seven Corners
Above are downloadable .pdf documents that explain the insurance coverage which includes limited health coverage, accident, and evacuation. Please read through completely. This is not a general health insurance policy, but an interim, limited benefit period, travel medical plan intended for use while away from your home country. The plan cannot guarantee payment to an individual or a facility for medical expenses until it has been determined that it is an eligible expense and a signed agreement has been received from the appropriate medical facility. You or the provider of service must contact the Assistance Company for pre-notification prior to any medical treatment. The Assistance Company has trained personnel available 24 hours a day, seven days a week to answer your questions, provide assistance and guide you to an appropriate facility if necessary. In case of an Emergency Admission, the Assistance Company must be contacted within 48 hours or as soon as reasonably possible. Pre-notification does not guarantee that benefits will be paid.
