*Our office will need to receive the Project Commitment and Roster Team Payment form with payment at least two weeks prior to departure.  Missioner Profiles will need to be mailed/emailed/faxed at a later date.

 

*Evaluations should be sent upon return.

 

*If you would like to email the forms, first: save the forms to your desktop, Fill out the forms, save again and attach to an email to our office. You can still print and mail/fax if you prefer.

 

USA Forms:

Team Member: all team members will need to fill out this form and return to the team leader.
