Effective January 1, 2011:

Please note that the cost for the range of days is the cost per person not a per day cost.
Mission Sports Insurance:
$10,000 – $2.25/per day per person $25,000 – $2.75/per day per person
Each person would also need to pay the following processing fee: Domestic Trips = $12/person. International Trips = $17/person.
Please complete this process 2 weeks prior to departure to avoid the $2 per person late fee.
Effective June 1, 2012
Below are links to the certificates that explain the insurance coverage which includes limited health coverage, accident, and evacuation. Please read through completely. This is not a general health insurance policy, but an interim, limited benefit period, travel medical plan intended for use while away from your home country. The plan cannot guarantee payment to an individual or a facility for medical expenses until it has been determined that it is an eligible expense and a signed agreement has been received from the appropriate medical facility. You or the provider of service must contact the Assistance Company for pre-notification prior to any medical treatment. The Assistance Company has trained personnel available 24 hours a day, seven days a week to answer your questions, provide assistance and guide you to an appropriate facility if necessary. In case of an Emergency Admission, the Assistance Company must be contacted within 48 hours or as soon as reasonably possible Pre-notification does not guarantee that benefits will be paid.
The Mission Sports coverage is the same basic coverage as the standard plus the Hazard Sports (motorcycle/motor scooter riding (whether as a passenger or a driver), hang gliding, parachuting, bungee jumping, water skiing, snow skiing, snowmobiling, snowboarding, and spelunking.)
The insurance company will email the ID card which has the toll-free numbers printed on it. The contact information for the Assistance Company is also on the certificate and the List of Frequently Asked Questions.
