How to register and insure your UMVIM, SEJ team (please follow exactly):

Note: Before registering your team with UMVIM, SEJ, you must complete Team Leader Training.  Click here for more information.

 

  1. Contact and register your team with your Conference Volunteer Coordinator.

  2. Download the Project Commitment form, the Team Roster form, and the Missioner Profile form.  The team leader must fill out the Project Commitment and Team Roster, and each team member must complete a Missioner Profile.

  3. Mail or fax all of these forms AT LEAST 2 WEEKS PRIOR TO DEPARTURE to:

  1. Mail a check (with abbreviated team information) to the above address, or pay online via Paypal.

  2. Before your departure, you should receive confirmation of your insurance from both UMVIM, SEJ and our insurance agent via email.

  3. For your safety and information, have your team members fill out the Medical Information and Release form, the Notification of Death form, and the Parental Consent form (if applicable).  Keep these forms with you on your trip.

  4. Have fun on your mission trip!

  5. Upon return, download and fill out the Finalized Record of Expense form, and have each team member complete a Team Member Evaluation form.  Mail or fax these to the above address.

  6. Plan another team, and go and serve again!
